One of the least known yet most profitable requirements for a well-managed brokerage is the obligation for any non-EU citizen to have private health insurance to obtain residency in Spain. This is not a recommendation. It is a legal requirement; without it, the application will not move forward.
For the broker who understands this mechanism, every residency application in their area is a concrete commercial opportunity. This guide explains exactly what the administration requires, what features the policy must have, and how to turn it into a recurring revenue stream.
Why does this requirement exist?
Spanish immigration regulations state that any non-EU citizen applying for a long-term residence permit must prove they have full healthcare coverage for the entire requested period. Applicants do not have access to the public healthcare system until they obtain residency and, in some cases, until they contribute to the Social Security system. Private insurance fills that gap.
Which types of residency require it?
Private health insurance is a requirement in most of the most common scenarios:
Non-lucrative visa
Aimed at European retirees and individuals with passive income. It requires private health insurance with zero copayments and full coverage nationwide. This profile represents high net worth individuals who often require multiple simultaneous policies.
Digital nomad visa
Created in 2023 for remote workers providing services to companies outside of Spain. It has the same insurance requirements as the non-lucrative visa. This is a young profile with high income, highly accustomed to purchasing services online.
Family reunification
Each reunited family member must prove healthcare coverage. A single family reunification case can generate multiple simultaneous policies for the brokerage.
Golden Visa
Investor profile with real estate or other assets in Spain. Health insurance is just one of the requirements, but these clients usually also need home, life, and commercial insurance.
What exactly must the policy comply with?
This is the point where many brokers make mistakes that can cost the client a visa denial. The Spanish Immigration Office does not accept just any health insurance.
Zero copayments
The policy cannot include copayments or deductibles. 100% coverage for consultations, tests, and hospitalizations.
No coverage limits
Coverage cannot be limited to a maximum amount per condition or per year. It must have a sufficiently high or unlimited cap.
Nationwide coverage
A policy with coverage restricted to a single autonomous community or a very limited network of centers is not valid.
Zero copayments
The policy cannot include copayments or deductibles. 100% coverage for consultations, tests, and hospitalizations
What documentation does the client request from the broker?
When clients approach the brokerage, they need more than just the policy. The administration requires specific documentation that the broker must know and prepare:
Complete policy including general and specific conditions
Valid payment receipt proving the policy is active
Certificate of coverage issued by the insurance company, sometimes required in English or with an apostille depending on the consulate
In some cases, a letter from the insurance company confirming that the policy complies with immigration requirements
The broker who anticipates this documentation and delivers it complete from the very beginning creates a client experience that stands out from those who simply sell the policy and leave the client to handle the paperwork alone.
What documentation does the client request from the broker?
Foreigners who need this insurance do not always know what to look for. Many arrive with incomplete information gathered from forums or groups in their own language. Some have already purchased the wrong policy and had it rejected by the Immigration Office.
A broker who masters this process—who knows which policy is valid and which is not, understands the requirements for each visa, prepares the complete documentation, and resolves doubts in the client’s language—does not need to compete on price
